Like their Facebook page, and hope the offer shows in your feed. The only way the form works is if you've already filled out a form on Facebook. Or you will not get the samples.

It lets you fill out the form, but I’m pretty sure it says that you have to already be signed up & in the UK. It asks on the first page if you are from USA, and once you click yes, it takes you to the page but it says 404 ERROR. Good luck, everyone. I don’t know if this one will work.
